The condition is severe physical stress — major surgery, burns, sepsis, critical illness — where the demand from rapidly dividing immune cells and gut-lining cells (enterocytes) can outpace what the body produces. Those two cell types use glutamine as a primary fuel source, more than glucose in some tissues. That biology is real. The question is whether it applies to someone training hard at the gym and eating normally.
Does Glutamine Build Muscle or Strength?
The most direct test is a double-blind trial by Candow and colleagues, who gave healthy young adults either glutamine or a placebo alongside six weeks of resistance training. Strength gains, lean body mass, and muscle protein breakdown markers came out the same in both groups. A later systematic review of glutamine for muscle strength recovery reached a similar conclusion: across the available trials, the evidence is too thin and inconsistent to recommend it for that purpose. A large randomized trial published in the New England Journal of Medicine gave critically ill ICU patients high-dose glutamine — far beyond a gym scoop, delivered enterally and intravenously under monitoring — and found no reduction in organ failure, with a trend toward higher mortality in the glutamine group. The takeaway for a healthy trainee is not "glutamine is dangerous." A 5-gram scoop after training and an ICU protocol are different worlds. The takeaway is that glutamine is not a harmless extra with no downside profile, and its clinical benefit is specific to people whose bodies are under genuine metabolic stress — not a default upgrade for anyone with a gym membership.
How to Use It If You Still Want To
If you are eating enough total protein and training normally, glutamine is unlikely to change your results either way. Where it is more plausible to make a small difference is a narrower list: very high training volume combined with a calorie deficit, frequent gut discomfort around intense sessions, or a diet that is genuinely tight on protein for a stretch.
